ELEVATING WORK PLATFORMS AND AERIAL DEVICES – EQUIPMENT INSTRUCTIONS AND MARKING
Cal. Code Regs, tit. 8, §§ 3638(d), 3203(a)(4) (2015) – Employer failed to instruct employees on proper use of an extensible boom platform prior to the employees operating the equipment.

INDUSTRIAL TRUCKS, TRACTORS, HAULAGE VEHICLES, AND EARTHMOVING EQUIPMENT – ELEVATING EMPLOYEES WITH LIFT TRUCKS, BRIDGE CRANES SHUT DOWN OR LOCKED OUT
Cal. Code Regs, tit. 8, § 3657(h) (2015) – Employer failed to shut down or lock out a bridge crane while an extensible boom platform was within the path of the bridge crane. The violation was classified properly as serious, accident-related; a willful classification was not supported.
