INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) - TRAINING ON NEW JOB ASSIGNMENTS
Cal. Code Regs, tit. 8 § 3203(a)(7)(C) (2011) - Employer’s employee was not fully trained regarding safe operation of lift gates. The Division established a general violation. 

MACHINERY AND EQUIPMENT - OPERATION UNDER STRESS
Cal. Code Regs, tit. 8 § 3328(a) (2011) - An employee operated a lift gate when it was loaded beyond its load capacity, which endangered employees. The Division established a general violation. 

TRANSPORTATION OF EMPLOYEES AND MATERIALS - SECURING LOADS
Cal. Code Regs, tit. 8 § 3704 (2011) - The load on a lift gate was not secured against dangerous displacement. The Division established a violation, but not that serious injuries were substantially probable in the event of an accident caused by the violation. The ALJ reclassified the violation to general.
