Code Regs, tit. 8, § 3648(o) (2011) - Two employees who were elevated over 19 feet in the bucket of an aerial device were not tied-off to the bucket or boom with lanyards. The violation, which was established by operation of law, was properly classified as serious, but the ALJ reduced the penalty.
