Labor Code §6619 (2015) – Employer’s petition for reconsideration did not include a proof of service indicating that it had been served on the Division.
PENALTIES – FAILURE TO REPORT SERIOUS INJURY
Cal. Code Regs, tit. 8, § 342(a), Labor Code § 6409.1(b) (2015) – An employee suffered a reportable workplace injury, which Employer did not report to the Division. The evidence did not show that Employer and a local fire department had entered into an agreement under which the fire department was to report the injury to the Division to fulfill both its own and Employer’s reporting obligations.
