Form 300A – Info Page

Form 300A is the summary of injuries and illness that employers must post in their workplaces every year from February 1st through April 30th. Additionally, most, but not all, employers have to upload their 300As electronically to a Fed-OSHA portal (details below).

Yearly Cal/OSHA Posting

California employers must post their summary of work-related injuries and illnesses in the workplace in the three-month period starting February 1st. The summary, which is based on information from the Form 300 log, must be placed in a visible and easily accessible area at each worksite. Current and former employees (and their representatives) are entitled to a copy of the summary, or the log upon request.

Employers are required to post the 300A summary even if they had no injuries or illnesses the previous year – they should just place zeroes though the relevant fields.

The posting requirement applies to employers with at least 11 employees.

Federal Portal: What’s Required and by Whom

There are two classes of employers who must, by March 2 of each year, submit their 300As to the Fed-OSHA ITA portal:

  1. All establishments or organizations with at least 250 employees;
  2. Any establishments between 20 and 249 employees.
