Employer, a city agency providing refuse collection services, was cited for two serious violations after an accident in which an employee's finger was amputated between a refuse collection truck and a refuse bin. Because Employer was a governmental agency, no penalty was proposed. Prior to the hearing, the Division withdrew one alleged serious violation for lack of evidence.
