LOS ANGELES -- The Division of Occupational Safety and Health has opened an investigation into a fatal explosion June 25 at Quion International, Inc., in Ridgecrest, Inyo County.According to Department of Industrial Relations spokesperson Susan Gard, two employees of a temporary agency, S.
