The Sept. 3, 2004, death of a construction employee at a Napa work site was due to poor communication between the work crew and the actions of the deceased employee, the Division of Occupational Safety and Health (DOSH) said in issuing a citation on the incident.DOSH cited the employer, Tri-County, Inc., for an alleged general violation of Construction Safety Orders 1592(e) and proposed a $300 penalty in the death of Rebecca Kogan, 39, of Menlo Park.Kogan was killed after being stru
