A serious violation of 1669(a) was established. Another serious violation (of 1646(a)) was established where Employer's employees worked on a scaffold 20 feet high which was neither tied off to a structure, guyed nor steadied by outriggers. A $100 penalty was affirmed for each violation. An alleged serious violation of 1646(b)(1) was dismissed.
