An accident investigation following a fatality at an electrical installation work site resulted in citation of Employer, an electrical contractor, for a general violtion of 2305.4(a) (use of unapproved messenger wiring to support insulated conductors in a three-phase system) and a serious violation of 2320.2(a)(3) (working without insulated gloves on energized conductors). The latter citation carried a $2,000 proposed penalty.
