Employer, a food processor, was cited following an accident in which an employee lost a finger tip when he attempted to lubricate a valve on a machine which filled 55-gallon bags with tomato paste. The Division cited a repeat/general violation of 3314(a) and proposed a $2,000 penalty because the employee failed to shut down and lock out the equipment before attempting the maintenance.
