An employee at Employer's wood products manufacturing site fell while descending a metal ladder fixed to the side of an elevated, rectangular metal storage bin. After conducting an accident investigation, the Division cited for serious violation of 3277(f)(4) and proposed a $2,000 penalty. Employer appealed the serious classification of the violation.
