Employer was manufacturing boiler parts. An employee lost the fleshy tip of his middle finger when it was pinched between a piece of stock and the outer vertical surface of the lower die of a mechanically powered press brake. The Division investigated, citing a serious violation of 4214(b)(9)(B) and proposing a $5,000 penalty.
