The Division conducted an accident investigation and cited Employer, a chemical manufacturer, for three serious violations, proposing penalties totaling $11,500. Prior to the hearing, the Division withdrew two of the allegations and their penalties of $1,300 and $3,750. It also amended the remaining serious violation from 5194(e)(1) to 5194(e)(2), carrying a $6,250 penalty.
