-EMPLOYER RECORDS OF OCCUPATIONAL INJURY OR ILLNESS – FORMS, BASIC REQUIREMENT
Cal. Code Regs, tit. 8, § 14300.29(a) (2014) – Employer did not fully complete the Form 300 for three calendar years.

PETROLEUM SAFETY ORDERS, DRILLING AND PRODUCTION – INJURY AND ILLNESS PREVENTION PROGRAM (IIPP)
Cal. Code Regs, tit. 8, § 6507 (2014) – Employer had in place procedures to identify and evaluate hazards; therefore, the first cited violation was not established. The injured employee’s testimony that he had never been trained on the use of the mud bucket involved in the accident supported the second cited violation.

MACHINERY AND EQUIPMENT – COMPONENTS DESIGNED AND SECURED OR COVERED TO MINIMIZE HAZARDS
Cal. Code Regs, tit. 8, § 3328(e) (2014) – The Division charged that Employer did not ensure that a mud bucket was properly secured to minimize hazards associated with a release of pressure from the tubing connection when it was broken. This was not the type of hazard contemplated by the safety order. The Division did not establish that the mud bucket was in place or in use on the tubing when the pressure release occurred.
