44 COR 40-7864 [¶22,719R] ELEVATING WORK PLATFORMS AND AERIAL DEVICES – EQUIPMENT INSTRUCTIONS AND MARKING Cal. Code Regs, tit. 8, §§ 3638(d), 3203(a)(7) (2017) – Employer did not provide instructions in the proper use of an extensible boom lift, as required by the safety order. INDUSTRIAL TRUCKS, TRACTORS, HAULAGE VEHICLES, AND EARTHMOVING EQUIPMENT – ELEVATING EMPLOYEES WITH LIFT TRUCKS, BRIDGE CRANES SHUT DOWN OR LOCKED OUT Cal. Code Regs, tit. 8, § 3657(h) (2017) – Employer failed to shut down or lock out a bridge crane while employees were working in an extensible boom platform that was within the path of the bridge crane. The violation was classified properly as serious, accident-related.
