HAZARDOUS ENERGY CONTROL – PROCEDURAL STEPS FOR SHUTTING DOWN, ISOLATING, BLOCKING AND SECURING MACHINES OR EQUIPMENT TO CONTROL HAZARDOUS ENERGY
Cal. Code Regs, tit. 8, § 3314(g)(1)(B) (2015) – Employer failed to establish procedural steps for shutting down, isolating, blocking, and securing the hazardous energy of a horizontal bandsaw prior to a cleaning operation, or that an exception to the safety order applied.

Cal. Code Regs, tit. 8, § 3314(j)(1) (2015) – Employer failed to provide the required training on hazardous energy control procedures or hazards related to cleaning, repairing, servicing, setting up, and adjusting a horizontal bandsaw.
PERSONAL SAFETY DEVICES AND SAFEGUARDS – HAND PROTECTION, GLOVES NOT WORN WHERE DANGER OF HAND PROTECTION BECOMING ENTANGLED IN MOVING MACHINERY OR MATERIALS
Cal. Code Regs, tit. 8, § 3384(b) (2015) – Employer failed to ensure that an employee did not use gloves where a danger existed of the gloves becoming entangled in moving machinery, a serious, accident-related violation.
