Reporting Violation With A Twist

California's Division of Occupational Safety and Health routinely cites employers for failing to report serious injuries or fatalities within the mandatory eight-hour period. But the Division also can cite police and fire departments for not reporting incidents they respond to, as demonstrated by a citation recently upheld by a Cal/OSH Appeals Board judge.
