A safety manager for Bovis Lend Lease was killed Feb. 25 in San Francisco after he fell 50 feet from scaffolding at a job site, according to the Department of Industrial Relations (DIR).The employee, identified as Ray O'Neal by DIR spokesperson Dean Fryer, had a "full body harness on when he fell," Fryer said.
