A Chino cabinet-making facility has been cited following three separate investigations by the Division of Occupational Safety and Health's High Hazard Unit (HHU) into several amputations on the same machine within five months.One of the amputations had not even been reported to Cal/OSHA, triggering a willful/regulatory violation of reporting requirements and a proposed $25,000 penalty.
