Violations, Serious, Willful, Defined. This appeal of an alleged "serious/willful" violation of 4191 was granted to the extent that it was reclassified as a "serious/repeat" and the proposed penalty reduced from $7,000 to $2,500.

The violation arose when an employee of Appellant, while under the direct supervision of Appellant's foreman, smashed her finger in an air press at the site.
