"An employee's hand was severely burned (and ultimately amputated) as she operated a machine to heat seal the edges of vinyl waterbed mattresses. A ram descended onto the material, heating it to 900 [degrees] F. to form a seam. Controls for the machine (intended for two-hand operation by two employees)
