Work Platforms, Elevated, Scissor Lift on Uneven Ground.

An employee was injured when he fell 40 feet off a mobile scaffold known as a "scissor lift" while painting. The lift was being moved and tipped when it came upon a depression two feet wide by three feet long and six inches deep.
