An employee was injured when a hardware box in which he was riding from roof to ground tipped and fell 20 feet or more from a forklift. The safety order requires that "(w)hen it is necessary to elevate employees using an industrial truck...(2) The platform shall be secured to the forks or mast to prevent tipping, slipping or falling."
