For Employer: Ordale B. Johnson, President. For DOSH: Jacqueline Schauer, Counsel.

Of five alleged serious violations, three were vacated and two (with total penalties of $495) were affirmed. All charges involved one ladder scaffold, from which an employee had fallen. It was made by placing a 2" x 10" x 8' plank, one end on the cross-piece above the topmost step of a stepladder, and the other end on a balcony. The plank was 11 feet, 4 inches above a concrete pad.
