A general violation of 1540(d) (protection in excavations) was established.
Employer was replacing valves in water lines. Its leadman entered a hole five feet wide, eight feet long and 10-1/2 feet deep with substantially vertical walls which had been shored until just before the inspection. The leadman had noticed a shoring jack that prevented the full taping of a valve and entered the unshored hole to correct the condition.
