A general violation of 1509(b) (accident prevention program) was affirmed.

Employer was laying gas transmission pipe for PG&E. As a truckload of pipe was being unloaded by crane, a 7,200-pound length of pipe rolled off the trailer and killed an employee. The Division's investigation suggested that some of the pipe lengths had been inadequately secured.
