A serious violation of 1621(a) (railings and toeboards) was affirmed, with $250 penalty. An employee was seen oiling forms on a building under construction (preparatory to another contractor's concrete pour) near the edge of a 28-foot roof section where standard guardrails were not in place. This exposed the employee to the risk of a 50-foot fall.
