An employee at Employer's sawmill fell while climbing a fixed ladder on a scaffold. A fire hose draped over his shoulder began to slip and he tried to grab it with one hand and lost his grip on the ladder with the other. Following an accident investigation, Employer was cited for serious violation of General Industry 3276(a)(1) and a $2,000 penalty was proposed.
