Employer was placing pipe along a highway. The Division conducted a permit inspection and issued citations for two general and two serious violations, with penalties of $350 and $35 for the latter. Twenty days later a drive-by inspection was conducted at the same site, resulting in citation for another serious violation and a proposed $700 penalty.
