Following an industrial hygienist's inspection, Employer was issued citations alleging a number of general violations and one serious violation with a proposed penalty. After a follow-up inspection, a Notice of Failure to Abate was issued for all but two general violations, and penalties totaling $54,100 were proposed. Employer appealed the failure-to-abate penalties.
