Following an injury, the Division conducted an accident inspection of Employer's heating and air-conditioning business and cited for one regulatory and two serious violations, proposing penalties totaling $6,050. At the hearing, the Division withdrew the regulatory and one of the serious charges, leaving on appeal one serious violation and its $5,000 proposed penalty.
