The Appeals Board affirmed in its entirety the ALJ decision of October 29, 1993 (Digest 18,422), upholding two serious violations and their penalties of $1,200 each.

The violations arose from an accident in which a worker, while changing a belt on a water booster pump, lost his balance and inadvertently hit the starter switch on the equipment. When the engine cranked his hand was caught in the pulley and he lost a finger segment.
