The Appeals Board affirmed the ALJ's decision on Docket No. 94-2565, regarding an alleged general violation of 3664(a)(22). The second Docket No., 95-4592, was the result of a subsequent appeal filed for violation of the same safety order. The Board consolidated the two issues. Both Employer and the Division filed petitions for reconsideration in the case.
