While Employer committed three violations, one of them serious, it was safety conscious and had never had a serious accident. Although it was not appropriate to grant extensive penalty relief, the penalty calculation rating for Likelihood was reduced, resulting in a lower penalty total. An installment schedule was granted for payment.
