PENALTY - FAILURE TO REPORT INJURY
Cal. Code Regs, tit. 8, § 342(a) (2005) - An employee injury was reported tardily and only by Employer's insurance company. Because Employer provided no financial information or evidence of other mitigating factors, the full $5,000 penalty was found reasonable.
