HAZARDOUS ENERGY CONTROL - FAILURE TO DE-ENERGIZE AND BLOCK MACHINE WHILE CLEANING
Cal. Code Regs, tit. 8, § 3314(c) (2010) - An employee failed to use lockout/tagout procedures before cleaning a conveyor machine. However, the ALJ reduced the penalty because the likelihood rating was insufficiently supported.

HAZARDOUS ENERGY CONTROL - SEPARATE STEPS FOR EACH MACHINE
Cal. Code Regs, tit. 8, § 3314(g) (2010) - Employer’s written hazardous energy control procedures did not contain separate procedures for each machine. However, the ALJ reduced the penalty because ratings for severity, extent and likelihood were unsupported.

PRIME MOVERS AND MACHINERY - CONVEYORS
Cal. Code Regs, tit. 8, § 3999(b) (2010) - The Division established a serious violation for an unguarded nip-point between a conveyor belt and pulley. However, the ALJ reduced the penalty because a medium rating for extent was unsupported.
