JOBSITE VEHICLES – SEAT BELTS
Cal. Code Regs, tit. 8, § 1597(h) (2016) – Employer failed to require the use of seat belts, in violation of the safety order. An employee did not wear his seatbelt while operating a utility vehicle.

ACCESS, WORK SPACE, AND WORK AREAS – FIXED LADDERS, LANDING PLATFORMS
Cal. Code Regs, tit. 8, § 3277(j)(2) (2016) – Employer failed to provide a landing platform where employees had to step more than 12 inches to gain access to a fixed ladder, in violation of the safety order.

FIRE PROTECTION – PORTABLE FIRE EXTINGUISHERS, SELECTION AND DISTRIBUTION
Cal. Code Regs, tit. 8, § 6151(d)(4) (2016) – Employer failed to ensure that the travel distance to a Class-B portable fire extinguisher from a diesel fueling area was 50 feet or less, in violation of the safety order.

TUNNEL SAFETY ORDERS – INJURY AND ILLNESS PREVENTION PROGRAM, DESIGNATED ON-SITE SAFETY REPRESENTATIVE
Cal. Code Regs, tit. 8, § 8406(f) (2016) – Employer failed to designate an on-site certified safety representative qualified to recognize hazardous conditions and certified by the Division, a willful general violation.
