Cal. Code Regs, tit. 8, § 3314(g)(2)(A) (2012) - Employer did not have a specific lockout/tagout procedure for a dicer involved in an injury to an employee, who had not locked out the dicer during the cleaning operation. No exception to the safety order applied.
