INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – TOOLBOX OR TAILGATE SAFETY MEETINGS
Cal. Code Regs, tit. 8, § 1509(e) (2015) – The Division did not establish that Employer failed to conduct tailgate safety meetings every ten days and keep written records of safety meetings.

SAFETY INSTRUCTIONS FOR EMPLOYEES – CODE OF SAFE PRACTICES (CSP)
Cal. Code Regs, tit. 8, § 1510(a) (2015) – Employer failed to train employees on job hazards, including hazards of trenches and excavations, heavy equipment, hazardous atmospheres and its CSP.

EMERGENCY MEDICAL SERVICES – PROVISION FOR OBTAINING EMERGENCY MEDICAL SERVICES
Cal. Code Regs, tit. 8, § 1512(e) (2015) – The Division did not establish that Employer failed to make available at the jobsite phone numbers of emergency medical facilities. 

HEAT ILLNESS PREVENTION – TRAINING
Cal. Code Regs, tit. 8, § 3395(f)(1) (2015) – Employer failed to provide heat illness prevention program training to several employees.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – RECORDS OF SCHEDULED AND PERIODIC INSPECTIONS
Cal. Code Regs, tit. 8, § 3203(b)(1) (2015) – Employer failed to maintain records of scheduled and periodic inspections or identify unsafe conditions and work practices at the jobsite.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – DOCUMENTATION OF SAFETY AND HEALTH TRAINING
Cal. Code Regs, tit. 8, § 3203(b)(2) (2015) – The Division did not establish that Employer failed to maintain records for training orientation and that those records did not include the names of training providers.

REQUIREMENTS FOR PROTECTIVE SYSTEMS – PROTECTION OF EMPLOYEES IN EXCAVATIONS
Cal. Code Regs, tit. 8, § 1541.1(a)(1) (2015) – Employer failed to provide employees with required protection from a potential cave-in while working in an eight-foot deep excavation with type-B soil.
