CONSTRUCTION SAFETY ORDERS - INJURY AND ILLNESS PREVENTION PROGRAM (IIPP)
Cal. Code Regs, tit. 8, § 1509(a) (2011) - Employer had an effective IIPP, trained its maintenance field staff on traffic control and on the responsibilities of a lookout for highway maintenance projects, and documented that training. The Division failed to establish a violation.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) - CODE OF SAFE PRACTICES (CSP)
Cal. Code Regs, tit. 8, § 1509(b) (2011) - Employer had a written code of safe operating practices that contained specific written procedures related to repair of potholes and traffic controls for that work, and for conditions involving non-errant vehicles. The Division failed to establish a violation. 

GENERAL SAFETY PRECAUTIONS - SURVEY OF SITE CONDITIONS
Cal. Code Regs, tit. 8, § 1511(b) (2011) - Employer’s maintenance supervisor for a potholing project failed to conduct a thorough survey of the site prior to the beginning of the work. The Division established a violation.

VEHICLES, TRAFFIC CONTROL, FLAGGERS, BARRICADES, AND WARNING SIGNS - TRAFFIC CONTROL FOR PUBLIC STREETS AND HIGHWAYS
Cal. Code Regs, tit. 8, § 1598(a) (2011) - Employer utilized a system of traffic controls in conformity with its manual. The Division failed to establish a violation.
