Code Regs, tit. 8, §§ 1509(a), 3203(a)(6) (2014) – Employer failed to establish and maintain an effective IIPP in neglecting to recognize and address the hazards associated with operating aerial lifts on a passenger platform under construction at a rail station, and to devise methods to remove and minimize the hazards. The citation provided Employer with adequate notice of the charge.
