OCCUPATIONAL INJURY OR ILLNESS REPORTS AND RECORDS – REPORTING OF OCCUPATIONAL INJURY OR ILLNESS, EMPLOYER Cal. Code Regs, tit. 8, §14001(a) (2020) – Employer failed to file a complete report of an injury that required medical treatment beyond first aid, a regulatory violation. CONSTRUCTION SAFETY ORDERS – INJURY AND ILLNESS PREVENTION PROGRAM (IIPP), CODE OF SAFE PRACTICES (CSP) Cal. Code Regs, tit. 8, § 1509(b) (2020) – The Division failed to establish Employer violated the safety order. Employer’s forklift-specific code of safe practices related to the use of powered industrial trucks with a boom attachment. CRANES AND DERRICKS IN CONSTRUCTION – OPERATION, DURING LIFTING OPERATIONS LOAD, BOOM OR OTHER PARTS OF EQUIPMENT SHALL NOT CONTACT ANY OBSTRUCTION Cal. Code Regs, tit. 8, § 1616.1(o) (2020) – Employer violated the safety order when a boom attachment contacted an obstruction in a way that caused falling material. The citation was reclassified to general due to lack of Employer knowledge.
