CONSTRUCTION SAFETY ORDERS – INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) Cal. Code Regs, tit. 8, § 1509(a) (2019) –  Employer failed to ensure that its employees adhered its rule that all employees tie off while working at six feet or higher elevation. Additionally, Employer failed to conduct an inspection and hazard analysis that would have addressed risks and hazards associated with using only one plank as the platform for erecting a tower. Employer, as a result, violated the safety order by failing to effectively implement and maintain its IIPP. CONSTRUCTION SAFETY ORDERS – SCAFFOLDS, GENERAL REQUIREMENTS, ERECTION AND DISMANTLING OF SCAFFOLDS AND FALSEWORK PERFORMED UNDER SUPERVISION AND DIRECTION OF QUALIFIED PERSON Cal. Code Regs, tit. 8, § 1637(k)(1) (2019) – Employer failed to provide supervision and direction of a qualified person while erecting shoring towers. CONSTRUCTION SAFETY ORDERS – SCAFFOLDS, GENERAL REQUIREMENTS Cal. Code Regs, tit. 8, § 1637(a) (2019) – Employer failed to provide sufficient scaffolding when its employees could not safely work while standing on a permanent or solid construction footing at least 20 inches wide, a serious violation.
