PENALTIES – FAILURE TO REPORT SERIOUS INJURY
Cal. Code Regs, tit. 8, § 342(a) (2015) – Employer failed to timely report a serious injury to the nearest Division office within eight hours, but made a late report.

INJURY AND ILLNESS REPORTING PROGRAM (IIPP)
Cal. Code Regs, tit. 8, § 1509(a) (2015) – The Division failed to establish that Employer did not have a written procedure or that it failed to follow its procedures for an accident investigation when an employee sustained a serious injury.

COMPRESSED GAS AND AIR CYLINDERS – STORAGE, HANDLING AND USE OF CYLINDERS, OXYGEN CYLINDERS IN STORAGE SEPARATED FROM FUEL GAS OR COMBUSTIBLE MATERIALS
Cal. Code Regs, tit. 8, § 4650(d) (2015) – Employer failed to ensure that oxygen cylinders in storage were separated from fuel gas cylinders by a minimum 20 feet or by a non-combustible barrier at least five feet high.

ERECTION AND CONSTRUCTION – REINFORCING STEEL AND OTHER PROJECTIONS, PROTECTION OF EMPLOYEES AGAINST HAZARD OF IMPALEMENT
Cal. Code Regs, tit. 8, § 1712(c)(1) (2015) – Employer failed to ensure that employees working at grade or at the same surface as exposed protruding steel anchor bolts located less than six feet above the working surface were protected from impalement. The Division established a willful, but not a repeat, violation.

ACCESS, WORK SPACE AND WORK AREAS – PORTABLE LADDERS, PROHIBITED USES
Cal. Code Regs, tit. 8, § 3276(e)(16)(C) (2015) – Employer willfully failed to ensure that portable step ladders were not used as a single ladder in a partially-closed position.
