GUARDING OF ENERGIZED PARTS – ELECTRIC EQUIPMENT OPERATING AT 50 VOLTS OR MORE
Cal. Code Regs, tit. 8, § 2340.17(a) (2014) – Employer’s employees were exposed to unguarded live electrical wires.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – REQUIRED ELEMENTS
Cal. Code Regs, tit. 8, § 3203(a) (2014) – Employer’s IIPP did not contain all required elements.

HEAT ILLNESS PREVENTION – TRAINING
Cal. Code Regs, tit. 8, § 3395(f)(1) (2014) – Employer did not provide employees with required training relating to heat illness and its heat illness injury and prevention plan.

HEAT ILLNESS PREVENTION – REQUIRED ELEMENTS
Cal. Code Regs, tit. 8, § 3395(f)(3) (2014) – Employer’s heat illness and injury prevention plan did not contain all required elements.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – EVALUATION AND IDENTIFICATION OF WORKPLACE HAZARDS
Cal. Code Regs, tit. 8, § 3203(a)(4) (2014) – Employer did not implement its IIPP to effectively identify and evaluate workplace hazards. It did not inspect and evaluate an off-road vehicle to adequately identify the hazards to which employees could be exposed when operating it. The Division established a serious violation.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – TRAINING ON WORKPLACE HAZARDS
Cal. Code Regs, tit. 8, § 3203(a)(7) (2014) – Employer did not implement its IIPP to adequately and effectively train employees on all hazards and hazardous operations present in the workplace, including the hazards associated with operating an off-road vehicle. The Division established a serious violation.

MACHINERY AND EQUIPMENT – IMPROPER VEHICLE MODIFICATIONS
Cal. Code Regs, tit. 8, § 3328(f) (2014) – Employer’s employees were exposed to the hazard of operation of a vehicle with modifications that were not authorized by the manufacturer or in accordance with good engineering practice. The Division established a serious, accident-related violation

PERSONAL PROTECTIVE EQUIPMENT – HAZARD ASSESSMENT AND EQUIPMENT SELECTION
Cal. Code Regs, tit. 8, § 3380(f)(1) (2014) – Employer’s employees were exposed to the hazard of operation of a vehicle without personal protective equipment provided by Employer, a serious, accident-related violation. The ALJ reduced the penalty.

TELECOMMUNICATIONS – VEHICLE-MOUNTED MATERIAL HANDLING DEVICES AND OTHER MECHANICAL EQUIPMENT
Cal. Code Regs, tit. 8, § 8610(c) (2014) – Employer’s employees were exposed to the hazard of operation of a vehicle that did not have a roll-over protective structure (ROPS), a serious violation.
