INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – FAILURE TO IMPLEMENT IIPP
Cal. Code Regs, tit. 8, § 3203(a) (2016) – The Division failed to establish that Employer did not effectively implement its IIPP, with respect to identifying, evaluating and correcting hazards, or ensuring that industrial trucks were operated safely.

PERSONAL PROTECTIVE DEVICES – HAZARD ASSESSMENT AND EQUIPMENT SELECTION, USE OF PERSONAL PROTECTIVE EQUIPMENT (PPE)
Cal. Code Regs, tit. 8, § 3380(f)(1) (2016) – Employer did not effectively assess workplace hazards that necessitated foot protection, nor did it supply properly fitted PPE to employees or require affected employees to use such PPE.

INDUSTRIAL TRUCKS, TRACTORS, HAULAGE VEHICLES AND EARTHMOVING EQUIPMENT – AUTHORIZED OR SAFE SPEED
Cal. Code Regs, tit. 8, § 3650(t)(9) (2016) – Employer failed to ensure that industrial trucks were operated in a safe manner by: (1) maintaining a safe distance from other vehicles; (2) keeping trucks under positive control at all times; and (3) ensuring that all established traffic regulations were observed.

FOOT PROTECTION – APPROPRIATE FOOT PROTECTION FOR EMPLOYEES EXPOSED TO FOOT INJURIES
Cal. Code Regs, tit. 8, § 3385(a) (2016) – Employer failed to require appropriate foot protection for employees who were exposed to foot injuries from falling objects or crushing or penetrating actions of industrial trucks.
