INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) - CODE OF SAFE PRACTICES (CSP)
Cal. Code Regs, tit. 8, § 1509(b) (2012) - Employer violated the requirement to have a written CSP. 

HEAT ILLNESS PREVENTION - WRITTEN PROCEDURES
Cal. Code Regs, tit. 8, § 3395(e)(3) (2012) - Employer did not have written heat illness prevention procedures for performing painting work outdoors. 

SCAFFOLDS - GUARDRAILS
Cal. Code Regs, tit. 8, § 1637(k)(6) (2012) - Employer’s scaffold did not have guardrails for work platforms on which employees were preparing a building for painting. 

LIGHT-DUTY EXTERIOR SCAFFOLDS - LEDGERS AND RIBBONS
Cal. Code Regs, tit. 8, § 1640(b)(2) (2012) - Employer failed to support its scaffolding platforms with ledgers and ribbons. 

LIGHT-DUTY EXTERIOR SCAFFOLDS - PLATFORMS
Cal. Code Regs, tit. 8, § 1640(b)(5)(A) (2012) - Employer failed to comply with requirements that scaffolding platform planks be constructed using 2-inch by 10-inch or larger material. However, the penalty was vacated as duplicative. 

EYE AND FACE PROTECTION - RISK OF EYE INJURIES
Cal. Code Regs, tit. 8, § 3382(a) (2012) - Employer did not dispute that an employee was working without eye protection where there was a risk of eye injuries from projecting nails. 

FINANCIAL HARDSHIP - NO PENALTY REDUCTION
Employer failed to establish financial hardship sufficient to warrant penalty relief.
