Cal. Code Regs, tit. 8, § 342(a) (2015) – Employer’s failure to report a serious injury required assessment of the full $5,000 penalty.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – IDENTIFICATION AND EVALUATION OF WORKPLACE HAZARDS
Cal. Code Regs, tit. 8, § 3203(a)(3) (2015) – Employer failed to implement and maintain a proper IIPP by conducting and recording safety inspections to identify and evaluate workplace hazards prior to a building maintenance project.

FLOOR OPENINGS, FLOOR HOLES AND ROOFS – COVER CAPABLE OF SUPPORTING 400 POUNDS OR TWICE WEIGHT OF EMPLOYEES, EQUIPMENT OR MATERIALS
Cal. Code Regs, tit. 8, § 3212(b) (2015) – Employer failed to secure a cover to prevent accidental removal or displacement, which exposed employees to the hazard of a floor hole.
