Cal. Code Regs, tit. 8, § 1509(a) (2012) - Employer’s IIPP lacked provisions essential to its overall safety program. The Division established a general violation.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) - EMERGENCY MEDICAL SERVICES PLAN
Cal. Code Regs, tit. 8, § 1512(i) (2012) - Employer did not have a complete written plan to provide emergency medical services.

HEAT ILLNESS PREVENTION - WRITTEN PLAN
Cal. Code Regs, tit. 8, § 3395(e)(3) (2012) - The Division did not establish that Employer lacked a complete heat illness prevention plan.

INDUSTRIAL TRUCKS - LOADS OF EXCESSIVE WIDTH, LENGTH OR HEIGHT
Cal. Code Regs, tit. 8, § 3650(l) (2012) - The Division did not establish that Employer failed to secure an excessively high load on a forklift.

POWERED INDUSTRIAL TRUCK OPERATOR TRAINING - REFRESHER TRAINING AND EVALUATION
Cal. Code Regs, tit. 8, § 3668(d)(2) (2012) - Employer did not provide periodic refresher training and evaluation of its forklift operator’s performance.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) - SCHEDULED PERIODIC INSPECTIONS
Cal. Code Regs, tit. 8, § 3203(b)(1) (2012) - Employees performed scheduled and periodic inspections, but did not make written records of the inspections. The Division established a violation. 

INDUSTRIAL TRUCKS - ALL PERSONS IN THE CLEAR
Cal. Code Regs, tit. 8, § 3650(t)(12) (2012) - The Division did not establish that Employer’s forklift operator, before moving the forklift, failed to look in the direction of travel and make certain that all persons were in the clear.
